I've created a new Extension on GitHub - can someone check it for me please?

(You can tell I'm a bit new to this.  ;) )

I have created an Extension here... https://github.com/marktompsett/JobLogReport
What it does is to provide a new report template to report on the Scheduled Job Log.  I did it because I keep getting WSOD and "Allowed memory size ... exceeded" errors when trying to View Log in Scheduled Jobs.  I get this on a number of sites that I look after so I created this custom report as a workaround.

I would appreciate a bit of feedback, like have I done this right?  Also, does it install and work OK?

Ultimately I want to publish the Extension in the Extensions Directory, but only after I am confident that I am doing things OK.
